Your Tufts Health Forward plan coverage has ended

Plan coverage with Tufts Health Forward (Commonwealth Care) has ended. As of January 31, members of this plan who haven’t applied for new coverage may no longer have health coverage.

You must reapply for coverage through the Health Connector. Most members are eligible for new, affordable health plan options, such as Tufts Health Direct (our individual and small-group plan). Tufts Health Direct also includes several silver-level ConnectorCare plans for eligible individuals and families.

To reapply:

       1. Go to MAhealthconnector.org.

       2. Create a new account or log in to an existing account.

       3. Submit an online application to learn about your eligibility (you should find out right away).

       4. Learn about your health plan options, including Tufts Health Plan – Network Health, enroll in a plan, and pay your premium (if you owe one).
