If you are eligible for unemployment benefits in Massachusetts, you should apply for health insurance benefits through the Medical Security Program.

• If approved for the Medical Security Program Direct Coverage plan, you will be automatically enrolled in Network Health Extend, our Medical Security Plan.
• If approved for the Medical Security Program Premium Assistance plan, you can get more information about your benefits and cost by calling the Medical Security Program at 800-908-8801, Monday through Friday, 8:30 a.m. to 4:30 p.m.

To apply for the Medical Security Program:

• Step 1
• Download, print, and complete the Medical Security Program Application for Coverage.  
  
• Step 2
• Get copies of any documents you may need.
  
• Step 3
• Mail your completed application and copies of any required documents in an envelope addressed to:
                                                  Medical Security Program
                                                  P.O. Box 146758
                                                  Boston, MA 02114-0020
  
• Be sure to include the correct postage

• Step 4
  • Watch your mail for a decision from the Medical Security Program about your eligibility.
