You can apply for Network Health in several ways:

• Complete our contact form and we’ll get in touch with you.
• Call us at 888-257-1985 and we’ll help you apply.
• Or, follow these steps:
  
  • Step 1.
    Download and fill out a Medical Benefit Request (MBR) application.
    Remember, we’re happy to help you apply for MassHealth or Commonwealth Care.
    If you would like some help filling out your application, call us at 888-257-1985.
  • Step 2.
    Print and complete a Permission to Share Information form.*
  • Step 3.
    Get the documents you need.
  • Step 4.
    Mail your completed application and paperwork.
    Be sure to include the correct postage. 
  • Step 5.
    Watch your mail for a decision from the state about your eligibility.
