Are you eligible for Network Health Together^®?

You may be eligible for Network Health Together, our MassHealth plan, if you:

Live in Massachusetts
Are a U.S. citizen, U.S. national, or eligible resident
Meet income limits

AND are one of the following:

A parent or adult caretaker relative living with children under age 19
Under age 19
Pregnant
Out of work for at least one year
Disabled according to the standards set by federal law

Are you eligible for Network Health Forward^®?

You may be eligible for Network Health Forward, our Commonwealth Care plan, if you:

Live in Massachusetts
Are 19 years old or older
Don’t have health insurance, and/or your employer does not offer you health insurance
Are a U.S. citizen, U.S. national, or eligible resident
You are not eligible for the Medical Security Program, Tricare, QSHIP, or the Fisherman’s Partnership
Have income equal or less than $32,496 per year for an individual (or $66,156 for a family of 4)
