You may be eligible for Network Health Forward, our Commonwealth Care plan, if you:

• Live in Massachusetts
• Are a U.S. citizen, U.S. national, eligible resident, or Resident with Special Status
• Are 19 years old or older
• Don’t have health insurance, and/or your employer does not offer you health insurance 
• You are not eligible for the Medical Security Program, Tricare, or QSHIP (Qualifying Student Health Insurance Program)
• Have income equal to or less than $33,516 per year for an individual (or $69,156 for a family of four).
