Our social care management program can support your Network Health patients with anything that could affect their health. Our social care managers can meet with your Network Health patients in a convenient place and help them:
- Apply for food stamps
- Ask for benefits like Supplemental Security Income (SSI) and Social Security and Disability Insurance (SSDI)
- Coordinate services with the Department of Transitional Assistance (DTA) and/or the Social Security Office
- Get information about utility-assistance programs
- Access emergency housing
- Get community services in addition to the services we offer
- Identify disability support groups
- Coordinate transportation to medical appointments
- Access behavioral health services
Your Network Health patients do not need prior authorization or precertification to work with a social care manager. To ask our social care managers to contact one of your Network Health patients, just contact us online or call us at 888-257-1985 with the following information:

- Your Network Health patient’s contact information
- Your contact information
- Request reason
- Requested service(s)
