Our asthma program can help you educate your patients about treatment and prevention. By addressing asthma proactively, we strive to keep our members with asthma, adults and children, as active and symptom-free as possible.

We have care managers available to work with you to develop a written action plan that outlines the steps your Network Health patient should take each day to manage their asthma. The program also features home visits and educational materials in English and Spanish.

Asthma education materials for your patients

Here is a good overview for your patients newly diagnosed with asthma:

Asthma action plans for adults and children can help you develop a personalized asthma action plan with your patient. The plan can help your patients know what to do if they experience flare-ups. By using the traffic light colors of green, yellow, and red to describe the severity of symptoms, the action plan guides your patient through steps to treat their symptoms.
