Disease management is our population-based approach to how we provide clinical and quality management of chronic conditions. We identify your Network Health patients who have these conditions, or you can refer one of your Network Health patients to us who you think will benefit from our disease management program. We then use disease-specific interventions to alter the disease’s course.
Our disease management program includes the following clinical areas:
Twice a year we send you a report listing the names of your Network Health patients with asthma or diabetes who are due or overdue for an important screening or prescription fill. These reports also include patients due or near due for other health screenings, such as cervical cancer screenings, mammograms, and well-child visits. We encourage you to respond to these reports by completing our fax-back forms and sending us information so we can update our records.
