How you can help
Network Health Together® (MassHealth) and Network Health Forward® (Commonwealth Care):
Your Network Health patients must renew their MassHealth or Commonwealth Care coverage and Network Health membership every year. When your Network Health patients need to renew their membership, they will get an Eligibility Review Form (ERV) in the mail. Please remind your patients to return their completed form to the state as soon as they can, or they risk losing their MassHealth or Commonwealth Care coverage AND their Network Health membership. To obtain a list of patients in your panel who are up for redetermination, please contact your provider relations representative.
Network Health Extend (Medical Security Program):
Your Network Health Extend patients must notify the Department of Unemployment Assistance (DUA) weekly of their continued unemployment status. Please remind your patients to update their status regularly with the DUA, or they risk losing their Medical Security Program (MSP) AND their Network Health membership.

Network Health Together
Your Network Health Together patients can call our Renewal Helpline at 877-849-0545 to get help with the renewal process. Our Renewal Helpline can get your patients a new ERV, help them fill out the form, and/or answer questions about the renewal process.
For questions about the status of their application or specific questions about eligibility, your Network Health Together patients can also call the MassHealth Customer Service Center at 800-841-2900 (TTY: 800-497-4648).
Network Health Forward
Your Network Health Forward patients can call our Renewal Helpline at 877-849-0545 to get help with the renewal process. Our Renewal Helpline can get your patients a new ERV, help them fill out the form, and/or answer questions about the renewal process.
For questions about the status of their application or specific questions about eligibility, your Network Health Forward patients can also call the Commonwealth Care Customer Service Center at 877-623-6765 (TTY: 877-623-7772).
Network Health Extend
Your Network Health Extend patients can call our MSP helpline at 888-257-1985 or MSP Customer Service at 800-908-8801 for questions about their plan.
