  • Select the Search by Name button
  • Select whether you are searching for a doctor or specialist, facility or hospital, pharmacy, or behavioral health and substance abuse location
  • Enter your doctor’s first and/or last name, or the name of the facility or hospital, pharmacy, or behavioral and substance abuse location
  • You can search all cities, enter your city or town, or enter your ZIP code
  • Click Search

To search by type:

  • Select the Search by Type button
  • Select whether you are searching for a doctor or specialist, facility or hospital, pharmacy, or behavioral health and substance abuse location
    • If searching for a doctor or specialist, choose type of doctor or specialty type
    • If searching for a hospital or facility, choose type of hospital or facility
  • You can search all cities, enter your city or town, or enter your ZIP code
  • You can also choose the Advanced Search option in the upper-right corner of the tool to select more criteria, including:
    • Hospital affiliation
    • Medical group affiliation
    • Language
    • Gender
    • Handicap accessibility
    • Accepting new patients
    • Plan Type
  • Click Search

Your results:

  • Your results will appear and will have an address, phone number, and office hours (as available) for each result. Click on more info (if available) to see more information about each result.
  • To print, select Print this page of results or Print all results

To conduct a new search, click on Quick Search or Advanced Search in the upper-right corner of the tool.

 

Source of information, validation and limitations

 

We receive information about practitioner and provider name, gender, medical group affiliation, hospital affiliation, office (or hospital) locations, languages spoken, and acceptance of new patients from each practitioner, provider, or hospital. We validate this information at a minimum of every two years during re-credentialing (or every three years for hospitals). We verify information on specialty and board certification (or accreditation for hospitals) at the time of initial credentialing and re-verify this information at least every two years (three years for hospitals). The accuracy of the information on our Web site and in our provider directory depends on the accuracy and timeliness of the information reported to us by the practitioners, providers, specialty medical boards, and hospital accreditation companies. We update any new information as we receive changes.

 

Definitions

 

  • Specialty – a specific area of medicine a provider is certified to practice, like cardiology, gastroenterology, oncology, or podiatry
  • Hospital affiliation – a hospital to which a provider can make referrals and may have admitting privileges
  • Medical group affiliations – a group of providers who practice together in the same community and who admit patients to the same hospital
  • Board certification – additional training in a medical specialty obtained by a provider, or special exams passed by a provider to prove competency in a specialty
  • Accepting new patients – when a provider is willing and able to treat new patients in their practice
  • Languages – languages that the provider or a provider's office staff speaks, in addition to English
  • Hospital accreditation – a "seal of approval" an external quality review company gives hospitals and facilities after reviewing the organization against the accreditation company's performance standards
