How to get your reimbursement
- Print the Reimbursement Form (PDF) or call us at 888-257-1985 to ask us to send you a reimbursement form.
- Make a copy of your receipts for yourself.
- Fill out the reimbursement form (be sure to make a copy for your records).
- Mail the completed form and the original receipts to:

Please note
- You must be a member when we process your reimbursement.
- Each Network Health Forward® member can get one $50 reimbursement every 12 months.
- You should get your reimbursement in 4 – 6 weeks.
