We know that maintaining your health and well-being sometimes requires a holistic approach, so we’re offering our Network Health Forward® members money back on acupuncture services.
Who will we reimburse for acupuncture services?
- All Network Health Forward members. You must be a member at the time of your appointment and when we process your reimbursement. Remember, we will reimburse you up to $150 every 12 months.
How to get your reimbursement
- Visit any licensed acupuncturist.
- Make a copy of your acupuncture receipt; be sure it shows the acupuncturist’s license number.
- Print the Reimbursement Form (PDF) or call us for one at 888-257-1985.
- Fill out the reimbursement form (be sure to make a copy for your records).
- Mail the completed form along with your receipts to:

Please note
- You must be a Network Health Forward member when we process your reimbursement.
- You should get your reimbursement in 4 – 6 weeks.
- Network Health Forward members can get one reimbursement of up to $150 every 12 months.
