Network Health Honored Nationally for Having a Top Care Management Program
Network Health’s care management program demonstrates integrated care excellence


(Medford, Mass.) — Network Health, a Massachusetts health plan that provides access to high-quality health care to more than 160,000 members with low and moderate incomes, today announced its receipt of a Case In Point Platinum Award for Best Overall Case/Care Management Program. Network Health bested hundreds of other health plans from across the United States in winning this award.

The Case in Point Platinum Awards recognize the most successful and innovative case/care management programs in the United States. Dorland Health, a leading health care publications company, sponsored the event and presented awards to health providers, plans, and programs that best educate and empower patients, improve wellness, and deliver sustained success. Health care experts from clinical and health care industries judged the competition.

Network Health’s nomination focused on how it empowers its members to manage their health care needs through an integrated, member-centered care management model. Network Health’s integrated care management model includes medical, behavioral health, and social care management, and delivers customized and coordinated care to its diverse membership. The model ensures continuity of care by fulfilling member needs along the continuum of care and gets support from a team of nurses, behavioral health care clinicians, and licensed social workers.

Network Health uses clinical best practices to evaluate its care management program and bases decision-making on data analysis and member needs. In FY09, Network Health’s integrated care management program achieved a 98% member satisfaction level.
